We Care Home Health is currently seeking a Fee For Service Physical Therapist in San Diego, CA!
Responsibilities:
Conducts a thorough and precise assessment of the patient's condition, functional abilities, homebound status, and environmental factors. Makes appropriate recommendations for necessary equipment, home modifications, and referrals to PT facilities and other therapies as indicated. Provides care in accordance with physician orders, the patient's condition, accepted standards of practice, Agency policies, procedures, and processes. Identifies changes in the patient's condition, consults with physician and/or Patient Care Coordinator (PCC) and implements appropriate interventions. Utilizes appropriate infection control techniques. Instructs patient, family and or/caregiver in all aspects of patient care. Participates in care planning. Develops and supervises Physical Therapy Assistant (PTA) and/or Home Health Aide (HHA).
Will service Ramona, Valley Center, and Fallbrook areas.
Will be working weekends only.
Qualifications:
* Free from Medicare Sanctions (required)
* Current PT License (required)
* Current CPR certification (required)
* Adequate means of transportation to facilitate timely visits (required)
* Minimum of 12 months experience as a PT, with Home Health experience (preferred)
