Individual will provide care in accordance with physician orders, Occupational Therapist direction, the patient's condition, and accepted standards of practice and Agency policies, procedures, and processes. Identifies changes in the patient's condition, consults with the OT, and implements appropriate interventions as directed by the OT. Will utilize individualized occupational therapy treatment plan to include education of the patient's physical condition. Will participate in and/or conduct case conferences, in-services, competency demonstrations/evaluations, and orientation as required.
Qualifications:
* Free from Medicare Sanctions (required)
* Current COTA license (required)
* Current CPR (required)
* Adequate means of transportation to facilitate timely visits (required)
* Minimum of 1 year experience as a COTA with Home Health experience (preferred)
