The Occupational Therapist must be licensed and/or eligible for licensure as required in the state of practice. He/she must be a graduate of an occupational therapy curriculum accredited jointly by the American Occupational Therapy Association and certified by the American Occupational Therapy Association under its requirements. He/she must be registered and/or eligible for certification by the American Occupational Therapy Certification Board.
The Occupational Therapy Assistant must be licensed and/or eligible for licensure, as required by the state of practice. He/she must be a graduate of an accredited Occupational Therapy Assistant Program certified by the American Occupational Therapy Association. He/she must be certified and/or eligible for certification by the American Occupational Therapy Certification Board.
The Physical Therapist must be a graduate of a physical therapy curriculum accredited jointly by the Council on Medical Education of the American Medical Association and the American Physical Therapy Association and certified by the American Physical Therapy Association under its requirements.
The Physical Therapy Assistant must be a graduate of an accredited Physical Therapy Assistant Program certified by the American Physical Therapy Association. He/she must be licensed and/or eligible for licensure as required by the state of practice.
The Speech Language Pathologist must be a graduate of an accredited Speech Language Pathology Program. He/she must have current or pending licensure in the practicing state and must hold Certification in Clinical Competency.
The Director of Rehab must be a graduate of a physical therapy curriculum accredited jointly by the Council on Medical Education of the American Medical Association and the American Physical Therapy Association and certified by the American Physical Therapy Association under its requirements.
